Meet with a cardiac registered nurse or a registered dietitian to discuss effective ways to manage heart disease at home.

This education session for Sharp Rees-Stealy and Sharp Health Plan members will cover ways to better manage heart disease at home. Topics will include heart failure; its causes, signs and symptoms; medication; follow-up care; and your role in controlling heart disease. Fluid restriction, weight monitoring and the importance of a low-sodium diet will also be discussed.
